Abstract

The Individualized Care Plan (ICP) is one of the main assessment tools developed within the residential geriatric setting. Its development is complex, due to the number of professionals who intervene in it, the different models of care that converge in the field of gerontology and the diversity of centers that make up the entire residential care. From the discipline of Social Work, this complexity is even greater, due to the great variety of aspects that can be evaluated that we find in the social field and the confluence of objective and subjective aspects typical of the social sciences.
Through this article, the fundamental aspects that intervene in the elaboration of the Individualized Care Plan from the perspective of Social Work are analyzed. Finally, a proposal for an Individualized Care Plan model is developed from the perspective of Social Work, unifying the aspects collected in the identified scientific literature related to the preparation of the ICP and the general principles of Social Work.
